Bernie's Physical Exam Results

HEENT:
Head is brachycephalic ; pupils equal and reactive. Brushfield spots present. Fundoscopic exam ____. Narrow nasopharyngeal area. Underdeveloped soft palate and enlarged tongue. Mouth breathing apparent. Enlarged adenoids and tonsils. Immature dentition.
Neck:
Low muscle tone, slight cervical lymphadenopathy
Chest:
Congestion upon auscultation. Regular heart rate and rhythm. Heart sounds indicate a slight murmur.
Abdomen:
Soft, nontender. Decreased bowel sounds.
Rectal:
Fissues revealed by routine rectal examination.
Extremities:
Simian crease, slight clubbing and cyanosis. Skin appears dry.
Neurologic:
Patient is awake and responsive, but somewhat lethargic. He smiles, but does not talk (babble) or attempt to crawl. Cranial nerves are intact. Motor exam showed decreased strength throughout arms and legs. Reflexes decreased. Some disturbances in coordination.
